- In office $200
Follow up appointments
- House call $100
- In office $80
(payment is due at the time of service)
Family, Group or Team rates are available when multiple patients are treated on the same visit. Please call for rate details.
Insurance:
- Insurance is not accepted for house calls. Upon request I can provide you the information needed for you to provide to your insurance company. Your reimbursement from your insurance company will depend upon your specific policy. Contact your insurance company for your coverage details.
- Insurance is accepted for in office appointments and may be submitted for you. For insurance companies that I am a out-of-network provider you will be responsible for the fees listed and payment is expected at the time of service unless other arrangements have been made.
